ACHA Revisions to ALF and Resident Health Assessment FORM 1823

Recently, Florida Agency for Health Care Administration (ACHA) made revision to the rules governing Assisted Living facilities (ALF).

There have been major revisions made to to the Form 1823 – resident health assessment. You can access the new form at Form 1823

There are also other items of importance. Such as OTC products can be used without a licensed health car provider’s order.

The new changes also make changes to the DNROs and amend the training requirement.

Is this form supposed to be filled out by the hospital or by the ALF the patient is going to?

The form has to be filled by a physician prior to or within 30 days of admission to an assisted living facility in Florida.
